Requirements

• Fully licensed at the Master’s level or above (LCSW, LICSW, or LCSCW).
• Valid National Provider Identifier (NPI) number.
• Active malpractice insurance.
• Only fully licensed providers are supported; candidates not yet fully licensed may reapply once licensed.
