Licensed Clinical Social Worker
About the role
The Licensed Clinical Social Worker (LCSW) at David Raines Community Health Centers provides counseling, evaluation, intervention, case management services to individuals dealing with mental, social, familial, and medical issues. This role involves examining the social and environmental factors impacting the client's life and providing community resources that can positively affect their lives.
Responsibilities
- Evaluating clients to determine their needs and the support options that can help them.
- Conducting psychosocial assessments and developing clinical treatment plans to address behavior or mental health needs.
- Performing suicide risk assessments, threat assessments, and providing crisis de-escalation.
- Coordinating services and resources, such as healthcare, community support, and financial aid, to meet clients' needs.
- Educating patients/families about disease treatment plans, including self-care post-discharge, holistic health needs, available resources, and follow-up care within the scope of the job.
- Utilizing critical thinking skills, decisive judgment, and working with minimal supervision.
- Working in a fast-paced environment and taking appropriate action.
- Performing other duties as required.
Requirements
- Master of Social Work (MSW) from an accredited institution.
- 2 - 5 years’ experience, preferably working with children and/or adolescents.
- Strong communication, crisis management, and interdisciplinary collaboration skills.
- Experience with electronic health records, preferably NextGen or similar EHR systems.
- Licensure/Certification: Active Licensed Clinical Social Worker (LCSW) license in the State of Louisiana and BLS (American Heart Association).
