Licensed Clinical Social Worker
About the Role
The Program Social Worker (SW) is assigned to a pediatric or adult medical program. The Program SW provides an orientation to program services and policies for the participant/legal guardian, completes psychosocial assessments, and serves as a liaison between the rehabilitation or therapy team and the participant/legal guardian. Responsibilities include providing updates on progress, projected discharge dates, identifying and referring to community resources, offering support and education to patients/legal guardians, consulting on mandated reporting guidelines, and acting as an intra-agency liaison for care. The SW also delivers in-services to teams at Ability KC related to social services as requested, follows agency regulatory guidelines, serves as a backup to other SW staff upon supervisor request, and performs other assigned duties.
Responsibilities
- Provide orientation to program services and policies for participants/legal guardians.
- Complete psychosocial assessments.
- Serve as liaison between rehabilitation/therapy teams and participants/legal guardians, providing updates on progress and projected discharge dates.
- Identify and refer participants to appropriate community resources.
- Provide support and education to patients/legal guardians.
- Consult on mandated reporting guidelines.
- Act as intra-agency liaison for care coordination.
- Deliver in-services to Ability KC teams on social services topics as requested.
- Follow agency regulatory guidelines for social work services.
- Serve as backup social worker to other staff upon supervisor request.
- Perform other duties as assigned.
Requirements
- Bachelor’s or Master’s degree in Social Work.
- LBSW or LMSW in Missouri in good standing (required); LMSW preferred.
- Minimum of one year of relevant direct care social service clinical experience.
- One year of experience providing social work services in a medical/rehabilitation or school setting (preferred).
Skills
- Knowledge of community resources in transportation, housing, healthcare access, financial assistance, and resources relevant to the population served.
- Understanding of family dynamics and issues related to adjustment to disability.
- Strong time management, prioritization, punctuality, rapport-building, team collaboration, and communication skills.
- Basic computer skills, including proficiency in Word and Excel.
