License Practical Nurse
Key Responsibilities
- Provides direct patient care as defined in the State of Nursing Practice Act
- Implements plan of care indicated by the clinician
- Provides accurate and timely documentation consistent with the plan of care
- Affords patient and family/caregiver education and information pertinent to diagnosis and plan of care
- Participates in coordination of home care services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, SLP) to the case manager or Clinical Supervisor
- Uses equipment and supplies effectively and efficiently
- Participates in personal and professional growth and development
- Performs other duties as assigned by the case manager or supervisor
Required Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Current state license as an LPN/LVN
- 1+ years experience as a nurse in a clinical care setting
- Demonstrates the ability to work collaboratively as part of an interdisciplinary team
- Excellent verbal and written communication skills
- Strong organizational and time management abilities
- Solid clinical judgement and the ability to follow care plans accurately
- Calm, compassionate demeanor
- Reliable transportation and valid automobile insurance coverage
Desired Qualifications
- Experience in home health or hospice preferred

Hours
8:00 am – 5:00 pm, Monday through Friday
Pay Range
$35 - $38 USD
