Responsibilities
- Manage client care efficiently and cost-effectively to achieve positive patient outcomes regardless of payer.
- Participate in the Home Health Aide Training Program (HHATP), teaching new staff and ensuring seamless transition from classroom to field.
- Develop the overall plan of care for clients and oversee its execution, including care delivered by LPNs, HHAs, and PCAs.
- Provide and coordinate quality patient care while maintaining an assigned caseload/productivity.
- Conduct admission assessments to determine safe plans of care, including the need for home health aides or personal care assistance.
- Case manage primarily aide-related services according to regulatory guidelines.
- Conduct in-home supervisory visits to oversee care given by home health aides and personal care aides.
- Practice according to best practice and regulatory nursing standards.
- Maintain relationships and clear communication with MLTC case managers and other care team members.
- Communicate effectively with aides, providing mentoring and positive feedback through phone conversations and visits.
- Oversee clinical skills of assigned aides and provide training as needed.
- Document care and communications according to standards of nursing practice.
- Contact primary physicians and notify clinical supervisors per regulatory guidelines.
- Mentor new hires with consistent oversight and provide leadership to Home Health Aides and Personal Care Aides.
- Review aide productivity, performance management, employee relations, and disciplinary actions.
- Assist in completing annual performance evaluations for aides.
- Investigate and address aide concerns, complaints, accidents, and other issues involving peers, patients, or care delivery.
- Report to Regional Director, Executive Director, and/or Human Resources as appropriate.
- Assist the clinical educator in developing aides' knowledge and skills to ensure proficiency and patient care quality.
- Ensure all assigned staff complete annual in-service credits.
- Assist in coordinating the HCR Preceptor program.
- Demonstrate competencies related to information and technology.
- Establish and maintain relationships with community partners.
- Perform other duties as assigned.
Requirements
- Current, valid RN license issued by NYS Department of Education.
- Minimum of two (2) years’ experience in home care.
Qualifications
Bachelor’s Degree in Nursing preferred.
Work Environment
This role is primarily in a non-office setting and may be exposed to outdoor conditions. The working conditions are classified as heavy work, involving exerting up to 100 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull, or otherwise move objects, including the human body. Heavy work involves sitting and standing.
