Responsibilities
- Oversight of daily operations of care management team, monitoring performance to meet KPIs
- Conduct regular check-ins and performance reviews to support effective member management
- Ensure timely care planning and execution to address member needs
- Collaborate with operational leadership to secure tools and resources for the team
- Facilitate cross-functional collaboration for seamless member care
- Lead teams through operational and clinical changes, gathering feedback for process improvements
Qualifications
- Active, unrestricted Registered Nurse (RN) license in good standing in the state of operation
- 3+ years of clinical experience
- 1+ years in a lead or supervisory role
- Ability to supervise daily activities and resolve departmental issues
- Strong interpersonal skills for team coaching and development
