Lead Physician, Outpatient Value Based Care

About the Role

AbsoluteCare is seeking a dynamic, mission-driven Lead Physician, Primary Care in Dayton to put the principles of team-based collaborative care and population health into action for our medically complex and socially vulnerable members. AbsoluteCare is designed to care for society’s most complex patients under innovative value-based care arrangements. Our patients have typically been underserved by the traditional health care system, resulting in frequent bouncing between emergency rooms, hospitals, and specialists. Our health care teams break the status quo by transforming our members’ experience to improve quality of life and overall health outcomes while reducing high costs of unnecessary utilization.

This exceptional care is provided by an integrated team of primary care providers, complex care managers, nurses, medical assistants, behavioral health clinicians, and community health workers. We offer complex, coordinated medical care with our onsite infusion center, full pharmacy, and lab & imaging suites. We also address the social determinants of health, including housing instability, food insecurity, and social isolation.

Responsibilities

The Lead Physician provides direct clinical care to a panel of members and guides clinicians on best practices for delivering value-based care. They work closely with the Medical Director to provide clinical leadership to providers, nurses, behavioral health specialists, and medical assistants.

• Provide excellent primary care to a panel of members, leading your pod interdisciplinary care team.
• Coach fellow providers in delivering excellent primary care, urgent care, or community care, and effectively lead their interdisciplinary care teams.
• Mentor care teams during population health huddles to achieve quality and utilization goals.
• Serve as a clinical leader in the center, addressing questions and providing guidance to nurses, medical assistants, and fellow providers about clinical cases.
• Drive the market toward achieving its quality, utilization, and engagement goals as part of the center and market’s leadership team.
• Act as AbsoluteCare’s ambassador to community-based organizations, hospital systems, and payer partners.
• Provide primary care services to a panel of members.
• Work in a team environment with social work, care management, behavioral health, nursing, pharmacy, and nutrition teams to facilitate and implement care plans.
• Coach and mentor physicians and advanced practice providers.
• Treat acute exacerbations of chronic conditions onsite.
• Accept same-day/next-day or urgent appointments for patients needing intensified physician or case management services.
• Determine when a higher level of care is needed for patient exacerbations.
• Forge partnerships with academic programs and community-based organizations to enhance member services.
• Prevent ER visits, hospital admissions, and readmissions through vigilant patient tracking.
• Practice evidence-based medicine in accordance with the PCMH model of care.
• Track and achieve specified quality metric goals.
• Implement care plans in collaboration with patients, their families, and other team members.
• Develop strong transitional care plans and track paneled patients across the continuum of care.
• Use technology and the electronic health record to increase quality initiatives and efficiency.
• Participate in multi-disciplinary meetings and recommend treatment initiatives.
• Provide on-call coverage for the office in rotation with other providers.
• Develop clinical protocols and pathways to upskill clinical support staff.
• Dedicate approximately 70-80% of time to direct clinical care.

Requirements

• Active, unrestricted medical license in the state of employment.
• Clinical and administrative experience leading interdisciplinary teams.
• Experience working for a clinic, community-based organization, or inpatient unit.
• Excellent interpersonal communication skills.
• Strong problem-solving, project management, and organization skills.
• Independent self-starter and critical thinker interested in population health, healthcare utilization management, and care model development.

Working Conditions

This job operates in a professional office environment and routinely uses general office equipment. The role may require wearing special uniforms such as lab coats or surgical scrubs, protective or safety attire like gloves and masks. Exposure to diseases and infections of patients occurs daily. Shared workspace with other staff may be required. Travel to any of our centers or corporate office may be necessary based on company needs.

Pay

Over $300K potential in compensation.
