Lead Outreach Worker

About the Role

The Lead Outreach Worker contributes to AHF by re-engaging clients formerly lost to care, coordinating and implementing specific interventions to access clients who have fallen out of care, and assisting them to return to care. The role involves utilizing strong experience in understanding the locality and the specific challenges facing this population, and working to find creative solutions to locate and re-engage these clients.

The Lead Outreach Worker will conduct ongoing retention meetings with healthcare center staff to ensure no gap in retention efforts and uphold radical cooperation across divisions. By combining knowledge of the Ryan White Care Act Program, the LOW will facilitate enrollment in the Care Management program when clients are re-engaged, working closely with Licensed Clinical Care Managers and Non-Medical Care Managers.

The LOW must attend regular Outreach-focused training meetings conducted by the Ryan White Program and will locate and contact clients compassionately to ensure successful linkage and retention. The role also includes participating in the Ryan White Harris County Planning Committee as a representative of AHF to fulfill contract requirements.

The Lead Outreach Worker and clinic staff will work as partners in re-engagement and retention efforts, directly contributing to AHF’s success in meeting the deliverables of the Ryan White contract and measures established by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services.

Responsibilities

• Re-engage clients formerly lost to care through targeted outreach and interventions.
• Coordinate and implement strategies to locate and contact clients who have fallen out of care.
• Conduct ongoing retention meetings with healthcare center staff to ensure continuity of care.
• Facilitate enrollment in the Care Management program for re-engaged clients.
• Attend Outreach-focused training meetings conducted by the Ryan White Program.
• Participate in the Ryan White Harris County Planning Committee as an AHF representative.
• Collaborate with clinic staff to enhance re-engagement and retention efforts.
• Review and investigate prior client history to implement effective action plans.
• Make phone calls and visit community sites to locate clients.
• Advocate for AHF’s initiatives and enhance the organization’s reputation in the community.
• Ensure viral suppression and retention in care for clients, contributing to a healthier population.

Requirements

• Associate degree required.
• Valid driver license, as the position requires driving duties.
• Able to travel within the service area 40% or more each week.
• Proficient outreach skills with knowledge of HIV/AIDS and current community resources.
• Understanding of HIV risk behaviors.
• Experience in conducting outreach to engage individuals in HIV care.
