Lead Nurse (Colorado Springs, CO / Hybrid)
Position Overview
The Lead Nurse is responsible for providing clinical leadership, oversight, and support for FreedomCare’s Colorado Care Management Team. This role supervises Care Managers while ensuring the delivery of safe, compliant, and person-centered clinical services. The Lead Nurse partners closely with Operations, Compliance, Quality, and cross-functional teams to oversee clinical workflows, documentation, regulatory compliance, and quality improvement initiatives.
Responsibilities
Provide day-to-day leadership, mentorship, and clinical guidance to the Colorado Care Management team.
Serve as the primary clinical subject matter expert (SME) for Colorado operations.
Support Care Managers with complex clinical cases, escalations, and changes in participant condition.
Review and approve care plans to ensure they accurately reflect participant needs, authorizations, and regulatory requirements.
Assist Operations leadership in planning, implementing, and evaluating new clinical programs and initiatives.
Ensure compliance with Colorado Medicaid regulations, contractual requirements, and FreedomCare clinical policies by overseeing caregiver qualifications and training records, service authorizations, care plan implementation, documentation standards, and critical incident reporting.
Monitor clinical documentation for accuracy, completeness, and timeliness.
Conduct routine chart audits and documentation reviews to maintain audit readiness.
Identify compliance risks, documentation gaps, and workflow improvement opportunities while implementing corrective actions as needed.
Participate in internal audits, quality improvement initiatives, and regulatory reviews.
Support investigation and follow-up of incidents, hospitalizations, complaints, and changes in participant condition.
Supervise, coach, and develop Care Managers through regular feedback, mentorship, and performance management.
Promote consistent clinical practices through education, competency development, and ongoing coaching.

Monitor day-to-day clinical operations by analyzing workflows, productivity, and quality metrics.
Develop and improve clinical workflows, standard operating procedures (SOPs), and best practices.
Support the implementation of new clinical programs, operational initiatives, and regulatory changes while ensuring successful adoption by the Care Management team.
Partner with Operations, Compliance, and other departments and external vendors to improve clinical operations and patient outcomes.
Monitor clinical capacity and assist in workload distribution to ensure timely patient services.
Qualifications
Active Registered Nurse (RN) license in good standing (Compact licensure to serve multiple states or willingness to obtain strongly preferred).
Bachelor of Science in Nursing (BSN) required, master’s degree in nursing, Healthcare Administration, Public Health, or related field preferred.
Minimum 3–5 years clinical nursing experience working with elders and/or adults with disabilities in home- and community-based settings (HCBS), long-term services and supports (LTSS), case management, or chronic disease management.
Minimum 2 years in a clinical leadership, supervisory, or program management role, preferably within Medicaid-funded programs.
Minimum 1 year of experience developing, writing, or implementing Standard Operating Procedures (SOPs) or similar policies, workflows, or clinical process documentation.
Experience supporting patients and caregivers in a homecare environment including caregiver coaching, care plan development, and long-term services coordination.
Strong regulatory compliance orientation with the ability to translate state requirements into internal workflows and training.
Experience serving as a subject-matter expert, escalation point, or clinical resource for staff.
Strong communication and interpersonal skills.
Proficiency in using office software (e.g., Microsoft Office suite).

Experience using a CRM (Salesforce is a plus).
