About the role

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is a non-profit provider of substance use and behavioral health programs in Arizona, including prevention, education, and treatment using evidence-based models.

The Lead Navigator is an individual in recovery from AOD use and/or GMH for at least one year with at least 90 days of experience as a Navigator or Peer Support Specialist. The primary role involves outreach and supportive services to assist individuals experiencing homelessness, coordinating and monitoring community-based services, and facilitating their delivery. The Lead Navigator mentors Navigator II staff, conducts outreach, engages clients, provides case management, and delivers wrap-around supportive services to individuals with high acuity and vulnerability.

Responsibilities

  • Conduct outreach and engagement with individuals experiencing homelessness.
  • Provide case management and ongoing wrap-around supportive services.
  • Coordinate and monitor RBHA and Non-RBHA community-based services.
  • Serve as a mentor to Navigator II staff, assisting with onboarding and program requirements.
  • Master processes including coordinated entry, housing referral, leasing up, wrap-around services, and discharge.
  • Work as part of a multi-disciplinary team including Patient Care Techs, Registered Nurses, Counselors, and Clinical Leads.
  • Coordinate care with community providers and stakeholders.

Requirements

  • High school diploma or GED required.
  • Associate degree or higher in a behavioral health-related field preferred but not required.
  • 1–3 years of full-time healthcare-related work experience providing exposure to behavioral and medical crisis situations preferred but not required.
  • Minimum of six months of recovery from substance use and/or mental health disorders required, or equivalent combination of education and experience.
  • Current Arizona driver’s license.
  • Clear 39-month motor vehicle record.
  • Behavioral Health Technician (BHT) certification in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures.
  • Arizona Fingerprint Clearance card, site-specific.
