About the role
The Lead Medicaid Unit Coordinator manages the financial eligibility, application, and recertification processes for residents utilizing Medicaid and Managed Long-Term Care (MLTC) programs. Serving as the primary liaison between the facility, residents, families, and state agencies, this role ensures all necessary asset verifications, budgets, and filings are submitted accurately and on time to prevent disruptions in funding.
Requirements
- High School Diploma
- Minimum 3 years of experience in a skilled nursing facility (SNF), Department of Social Services (DSS), or related field
Responsibilities
- Conduct pre-admission interviews with prospective resident family members and guide them through the Medicaid application process
- Collect necessary verifications (bank statements, income, assets) and submit filings, conversions, and yearly recertifications on time
- Review all documentation, including 60 months of banking, to determine Medicaid eligibility or penalty periods incurred
- Contact case workers at hospitals, Medicaid offices, or attorneys to follow up on Medicaid applications
- Maintain a chronological listing of all contacts and follow-up calls
- Process Medicaid applications with residents, families, attorneys, and respective Medicaid offices in a timely and professional manner
- Process recertifications for the appropriate county and follow up with residents and families
- Process new, corrected, and revised budgets accurately and timely with related parties
- Review admission logs to identify new Medicaid pending applicants on a timely basis
- Prepare and process Medicaid Pick-Up Statements efficiently and accurately
- Process new, revised, and corrected Pick-Up dates when needed, and follow up with attorneys, case workers, and elder care agencies to notify them of any changes
- Conduct face-to-face interviews with family and representatives while upholding a high level of professionalism and efficiency
- Prepare and submit all required, applicable 3559 and MAP forms timely
- Perform monthly reviews of Medicaid rosters
