About the Role

The Lead Direct Care Professional provides oversight, support, and direct care to individuals served by Hope Network across a variety of settings. This role serves as a leader on shift, modeling person-centered support, promoting independence, and ensuring the implementation of medical, clinical, therapeutic, and behavioral support plans. The Lead Direct Care Professional also mentors, trains, and orients new staff, assists with on-call responsibilities, and provides guidance and support to team members when supervisors are unavailable or off-site.

Responsibilities

  • Regular and predictable attendance.
  • Successfully complete all required and applicable trainings and in-services including annual refresher and appropriate intervention techniques.
  • Provide leadership, direction, and support to staff on shift in coordination with the supervisor, including scheduling, training, compliance, on-call pager, and administrative supports.
  • Elevate employee relations concerns to supervisor and assist with special projects as needed.
  • Provide input in the development of the consumer’s treatment plans and goals.
  • Confirm treatment plans are implemented properly.
  • Implement and participate in the clinical and behavioral goals for each consumer using approved methods of instruction and intervention.
  • Provide thorough, accurate, and on-time daily observational data and document using electronic records according to Agency standards.

Qualifications

  • Minimum Education required:
    • Bachelor’s degree with 2 years experience in applicable setting, or
    • Associate’s degree with 3 years experience in applicable setting, or
    • High School Diploma/GED with 4 years experience in applicable setting.
  • Demonstrated critical thinking/problem-solving skills to respond appropriately to medical and behavioral issues.
  • Demonstrated leadership skills and ability to problem solve independently.
