About the Role
Lead Direct Care Professionals at Hope Network provide oversight, support, and direct care to individuals across various settings. This role serves as a leader on shift, modeling person-centered support, promoting independence, and ensuring the implementation of medical, clinical, therapeutic, and behavioral support plans. You’ll mentor, train, and orient new staff, assist with on-call responsibilities, and guide team members when supervisors are unavailable or off-site.
Responsibilities
- Regular and predictable attendance.
- Complete all required and applicable trainings and in-services, including annual refreshers and intervention techniques.
- Provide leadership, direction, and support to staff on shift in coordination with the supervisor (e.g., scheduling, training, compliance, on-call pager, and administrative tasks).
- Elevate employee relations concerns to the supervisor and assist with special projects as needed.
- Provide input in the development of consumer treatment plans and goals, and confirm their proper implementation.
- Implement and participate in clinical and behavioral goals for each consumer using approved methods.
- Document thorough, accurate, and on-time daily observational data using electronic records according to agency standards.
Requirements
- Minimum education and experience:
- Bachelor’s degree with 2 years of experience in an applicable setting, or
- Associate’s degree with 3 years of experience in an applicable setting, or
- High School Diploma/GED with 4 years of experience in an applicable setting.
- Demonstrated critical thinking and problem-solving skills to respond appropriately to medical and behavioral issues.
- Demonstrated leadership skills and ability to problem-solve independently.
