Lead Care Manager - LCSW (Licensed Clinical Social Worker)

JOB SUMMARY

Full Time, 12:00 PM to 08:30 PM
Yearly Base Salary Pay Range: USD $94972 to $150321

REQUIRED QUALIFICATIONS

• Master's Degree in Social Work from an accredited program
• California Clinical Social Worker License upon hire
• 2 years of experience in healthcare related field (Acute, Ambulatory, or Post-Acute)
• 2 years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers
