Lead Advanced Practice Clinician - Cardiac Surgery - Jefferson Health- Philadelphia

An Advanced Practice Clinician (Nurse Practitioner/Physician Assistant) in the acute care hospital setting collaborates as an integral member of a multidisciplinary health care team, providing comprehensive care on the inpatient cardiac surgery service.

Responsibilities

• Performs complete history and physicals, documenting in the medical record.
• Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information, and diagnostic procedures.
• May serve as the first responder for both nursing and house staff in the event of patient status changes, orders, and situations requiring procedural and resuscitative interventions.
• Assesses risks associated with the care of acute and complex chronically ill patients, including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions, and diagnostic procedures.
• Collaborates with the interdisciplinary health care team in making diagnoses of acute and complex chronic conditions.
• Manages diagnostic tests through ordering, interpretation, performance, and supervision.
• Formulates differential diagnoses by priority and diagnoses complications, ordering appropriate interventions.
• Identifies expected outcomes from diagnosis and formulates a plan of care to address complex acute and chronic health care needs.
• Utilizes evidence-based practice guidelines and protocols in an individualized, dynamic plan of care applicable across the continuum.
• Implements and modifies the plan of care, prescribing diagnostic strategies and therapeutic interventions.
• May perform advanced procedures consistent with privileges and competency validation.
• Maintains ongoing communication and collaboration with the interdisciplinary health care team.
• Facilitates and communicates with patients, families, and staff to promote continuity of care across the inpatient and outpatient continuum.
• Documents key components of patient progress via daily progress notes, transfer and discharge summaries, and clinic notes (H&P, daily progress notes, plan of care, problem lists, procedure notes, acute event notes, discharge summaries, death summaries) in the medical record per specific patient unit or service, ensuring timeliness, compliance, and acuity capture.
• Demonstrates professional practice behaviors, including precepting/mentoring, education, and instruction of graduate students.
• Seeks opportunities for active engagement in research and analysis of evidence-based practice.
• Actively participates in Advanced Practice Grand Rounds and specialty-specific meetings.
• Maintains CEUs and membership in a professional organization.

Requirements

• Master’s Degree
• National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant
• BLS certification
