About the role

Supports individuals and families navigating emotional, behavioral, and life changes due to neurological conditions. Focuses on real-world psychosocial support delivered in home and community environments.

Responsibilities

• Performs comprehensive evaluations, assessing social, emotional, and support needs of patients and their families.
• Formulates measurable objectives in collaboration with the rehab team.
• Assists physicians and other health team members in understanding significant social and emotional factors related to patient health problems.
• Instructs and counsels patients and families on treating and coping with social and emotional responses connected with illnesses.
• Participates in care coordination activities and acts as a resource to other health team members in identifying and resolving patient needs.
• Identifies and assists patients and/or families in utilizing appropriate community resources to achieve measurable objectives.

Qualifications

• Master's Degree in Clinical Social Work (LCSW)
• Current Licensed Clinical Social Worker Licensure in the state of practice
• Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury, or acute diagnosis
• Current BLS (Basic Life Support), First Aid, and CPR certification required
• Ability to lift up to 100 pounds following safety procedures
• Fine motor skills, visual acuity, and frequent walking/standing required
