About the role
We are looking for a dependable, dedicated, and respectful Bilingual Lay Health Educator to assist with a patient health education program within our community health clinic. This role focuses on Diabetes, Metabolic Syndrome, and other chronic health concerns, following patient action plans based on physician assessments, conducting individual and group health education sessions, maintaining accurate records, and performing follow-up activities.
Responsibilities
- Assess patients’ current disease self-management behaviors, openness to adopting new behaviors, and physical, cultural, psychosocial, and economic barriers to change.
- Collaborate with health care providers and staff to formulate an education plan addressing identified goals and objectives.
- Implement the education plan using established principles of teaching-learning theory and lifestyle counseling.
- Evaluate patients’ understanding and utilization of self-management skills and knowledge, including reassessment of needs.
- Properly document all education encounters.
- Conduct regular health education classes for patients following an established curriculum.
- Maintain accurate records of day-to-day implementation activities to aid program evaluation.
- Foster communication of health needs, concerns, and resources between patients and providers.
- Initiate and tally patient satisfaction tools.
- Assist with the collection of data for quarterly reports.
- Adhere to the Corporate Compliance program by reporting improper or unethical conduct, violations of applicable laws, regulations, or program requirements.
- Perform all other duties as assigned.
Requirements
- Knowledge of medical terminology.
- 2 years of previous clinical experience as a Medical Assistant (MA) preferred.
- Previous teaching experience is an advantage.
- Bilingual in English and Spanish (required).
Skills
- Excellent interpersonal and communication skills.
- Ability to work varied hours, including evenings and Saturdays when necessary.
- Familiarity with Windows-based computer programs and desktop PC use.
- Knowledgeable in continuous quality improvement concepts, tools, and applications.
