About the role
TLC Management is seeking a dedicated and compassionate individual to join our team as a Resident Care Coordinator. The ideal candidate will have a strong background in healthcare and a passion for providing exceptional care to our residents.
Responsibilities
- Develop and implement care plans that meet the needs of residents
- Monitor resident health and wellness, reporting issues to appropriate staff members
- Facilitate communication between residents, families, and healthcare providers
- Coordinate activities that promote social interaction and mental stimulation
- Ensure compliance with all regulatory standards and policies
Requirements
- Bachelor's degree in Healthcare Administration, Nursing, or related field
- Minimum 2 years of experience in a healthcare setting
- Current CPR certification
- Excellent interpersonal and communication skills
- Ability to work flexible hours including evenings and weekends
Qualifications
- Strong organizational and time management skills
- Proficient in Microsoft Office Suite
- Experience with electronic health records (EHRs) preferred
Skills
- Knowledge of long-term care facilities and services
- Ability to handle sensitive and confidential information
- Positive attitude and ability to work in a team environment
