Position Overview
The Case Manager is responsible for providing the required assessment, case management, and counseling services to a designated caseload of clients. The Case Manager works collaboratively with each client to assist them in identifying and securing community-based housing as rapidly as possible.
Essential Duties & Responsibilities
- Provides case management to a designated caseload of clients.
- Conducts intake, psychosocial and housing assessments of clients.
- In collaboration with each client, prepares and monitors an Independent Living Plan, identifying short- and long-term objectives to be achieved.
- Inputs all client information and updates into the CARES system.
- Counsel clients through individualized and group sessions on issues such as well-being self-management and independent living skills.
- Identifies and makes referrals for community-based treatment and support services.
- With clients’ authorization, coordinates care with community-based providers.
- Works collaboratively with the Housing Specialist to identify and reinforce each client’s housing plan.
- Coaches/prepares clients for housing interviews.
- Collaborates with on-site psychiatric provider, nursing, and primary care to ensure each client has an appropriate aftercare plan.
Qualifications
- All candidates must have at least a Bachelor’s degree from an accredited college or university.
- Experience Requirements: A minimum of three (3) years of experience working with mentally ill individuals or homeless individuals with either a diagnosis of mental illness or a substance use disorder is preferred.
