- Required Medical Degree from an accredited college or university
- Diploma for completion of an American Board of Neurology accreditation internship and residency
- NBME or Flex certification
- American Board of Neurology certification or eligibility
- Unrestricted DEA number
- Unrestricted Georgia medical license
