About the Role

Join our nursing team as a Licensed Practical Nurse (LPN) and profoundly impact the lives of our residents by promoting, restoring, and maintaining their health and well-being. Collaborate with healthcare providers, the interdisciplinary team, and care associates to deliver compassionate physical and psychosocial support to residents and their families.

Responsibilities

• Develop relationships with residents and their families using a consistent caregiver approach, supporting physical, spiritual, emotional, and clinical needs while prioritizing the resident.
• Assess, observe, and evaluate residents' clinical conditions, document findings in the electronic medical record, and notify providers and/or RNs as necessary.
• Update or modify care plans to reflect changes in residents' care and service needs.
• Supervise and lead care associates.
• Administer medications timely, accurately, and according to orders.
• Participate in meetings related to resident outcomes and care.
• Respond to residents, families, or responsible parties in a timely and professional manner.
• Participate in the review of clinical indicator reviews (e.g., falls, wounds, nutrition, antipsychotic use).
• Perform other duties as assigned by the RN clinical leader.

Requirements

• Minimum of 1 year of experience in assisted living, home health, or long-term care (preferred).
• Licensed by the state as a Licensed Practical Nurse (LPN).
• CPR certification required.
• Passion for caring for seniors and strong resident advocacy.
• Experience with electronic medical records (preferred).
