About the role

The Care Facilitator is responsible for providing primary care of Intensive Outpatient clients including supervision and daily living services and ensuring the delivery of high-quality therapeutic services for each client. Tasks are performed under the direction of the VP of Lotus Clinics & Clinical Compliance.

Responsibilities

  • Supervise Intensive Outpatient clients to ensure the daily physical and emotional needs of the youth are being fulfilled.
  • Assist the Intensive Outpatient Director and therapeutic case coordinators with the overall operation of Intensive Outpatient services.
  • Assist in ensuring compliance with all COA standards and ensuring continuous quality improvement.
  • Monitor and maintain high quality record keeping and professional accountabilities for Intensive Outpatient Services.
  • Assist with contacting parents for follow up and review of their child’s progress in the Intensive Outpatient Program.
  • Assist in professional growth maintenance.
  • Utilize the Philosophy of Care of Phoenix and trauma-based interventions as directed.
  • Perform other duties as assigned or requested.

Requirements

  • High school diploma or equivalent.
  • College credit hours in social work, psychology, or a related mental health field from an accredited college, preferred.
  • Must be at least 21 years of age with a valid driver’s license and satisfactory driving record.
  • Excellent interpersonal skills and the ability to establish trust, confidence, and mutual respect.
  • Able to work independently as well as collaborate effectively in a team.
  • Strong organization and prioritization skills.
  • Able to adapt and be flexible.
  • Strong problem solving and judgment skills.
  • Excellent verbal and written communication skills.
  • Able to accept and discuss cultural differences and respond to culturally based cues.
  • Aptitude to work with youth who have emotional and behavioral challenges.
