Investigator - Incident Resource Manager
Key Responsibilities
- Conduct comprehensive investigations of reportable incidents, including interviews, fact-finding, and documentation.
- Collaborate with internal and external stakeholders including OPWDD and the NYS Justice Center.
- Prepare clear, timely, and objective reports in compliance with OPWDD Part 624/625 and Justice Center requirements.
- Support root cause analysis and recommend corrective and preventive action plans.
- Maintain confidentiality and high professional standards at all times.
- Participate in agency committees (e.g., Incident Review, Human Rights, Informed Consent) as assigned.
- Conduct follow-up and post-investigation monitoring to ensure implementation of recommendations.
Qualifications
- Bachelor’s degree in Human Services, Criminal Justice, Psychology, Social Work, or a related field (Master’s preferred).
- Minimum of 3 years of experience conducting investigations or managing incidents in a human services setting.
- Strong understanding of OPWDD regulations, NYS Justice Center standards, and incident management best practices.
- Excellent interviewing, analytical, writing, and communication skills.
- Ability to manage multiple priorities and meet deadlines.
- Experience with IRMA, electronic health records, and investigation tracking tools preferred.
- Flexibility to travel to sites and participate in on-call coverage as needed.
Work Environment
- This position is hybrid with a minimum of 3 on-site days per week in a primarily office-based environment.
- Regular travel to program locations across the five boroughs is expected.
- The role may involve remote documentation, on-call responsibilities, and in-person presence during critical incident reviews.
Position Type/Expected Hours of Work
- This is a full-time position, typically Monday through Friday, 9 a.m. to 5 p.m.
- Flexibility is required to address incidents or participate in reviews outside of core business hours.
