Interventionist - Lewis County

About the role

This position operates in Lewis County and serves children and families in Jefferson County who are at risk of psychiatric hospitalization or admission to group treatment programs.

Responsibilities

  • Respond to each referral within 48-hours.
  • Meet with each youth and family/guardian to discuss program options, service provision, and alternative service options.
  • Conduct all admission paperwork and ensure all necessary consents/releases are signed.
  • Collaborate with already established providers such as care managers, therapists, and psychiatric providers.
  • Provide intensive crisis intervention and supportive counseling to the youth and family.
  • Participate in team meetings.
  • Work with community agencies, organizations, and systems for long-term planning and support of the youth and families served.
  • Make referrals to mental health services and other programs as needed.
  • Provide thorough documentation regarding all case related activities.
  • Represent CHJC as a liaison with various private as well as public service providers and resource agencies.
  • Complete all CHJC and New York State mandated reports and forms in a timely fashion.
  • Promote the program and provide community education.
  • Participate in an after hours on-call rotation.
  • Travel to recruitment events, local schools, conferences and/or meetings and moderate travel within the community to include off-site/in-home visits.

Requirements

  • Required Education and Experience: Bachelor’s Degree in a Human Services field, with at least two years relevant experience in community-based mental health or case management is required. Preference given to those with a Master’s Degree and/or licensed clinicians.
  • Skills/Abilities/Knowledge: Strong organizational abilities and office skills, excellent people skills and teamwork, effective and excellent oral and written communication, detail-oriented, self-directed, tactful, diplomatic, clear, courteous, and professional, able to react to change productively, and handle other duties as assigned.
  • Additional Eligibility Qualifications: Must maintain privacy of confidential information, successful completion of routine pre-employment requirements, ongoing clearances and exclusion checks, employment health requirements, and corporate compliance training.

Special Requirements

  • Work Environment: Office environment with occasional travel, moderate travel within the community, and exposure to inclement weather conditions.
  • Equipment: Required to maintain a working knowledge of related office equipment.
  • Physical Demands: Must be able to sit, stand, walk, lift, carry, push/pull, climb, bend, and stoop. Must be able to perform fine motor skills, read, and type. Must be able to sit for long periods of time.

Pay

Starting rate dependent on licensure.

Schedule

Full-time, traditional hours and non-traditional hours. This position is required to work on-site, and offsite/in home visits. This position will be in the on-call rotation.
