About the role
The Division of Cooper General Internal Medicine is a part of Cooper University Health Care and provides Adult Primary Care to the communities of Southern New Jersey and physician services to Sub-Acute and Nursing facilities in the South Jersey area. We are a large Academic division of about 80 health care providers comprising Physicians and Advanced Practice Professionals located at sixteen clinical sites in South Jersey. We cover a large geographical area including practices within 30 minutes to Philadelphia, Wilmington, and Trenton. Division faculty are actively involved in the Internal Medicine Residency Program at Cooper Health and medical student education for Cooper Medical School at Rowan University. Our providers participate in an Accountable Care Organization with a population health team to support our providers. We have a dedicated team of Primary Care Providers with a shared mission of to “To Serve, To Heal, To Educate.”
Responsibilities
- Provide direct patient care in adult primary care outpatient settings- Acute visits
- Annual wellness visits
- Post-hospital discharge follow-up visits
- Deliver nursing home and sub-acute care, which may include:- New admissions
- Weekly and/or monthly follow-up visits
- Maintain flexibility to travel to multiple South Jersey clinic locations within the Division of Internal Medicine
- Be available to provide coverage at any affiliated nursing home service location
In-Basket & Results Management
- May be asked to cover in-basket tasks for providers being covered
- Responsible for managing:- Test results
- Patient messages
- Medication refill requests for patients the physician has personally seen or for whom orders were placed
Scheduling & Coverage Expectations
- Flexibility is required to respond to short-notice coverage needs, including:- Same-day requests
- Evening or night-before requests
Education Requirements
- Medical Degree - MD or DO
- Completion of an ACGME-accredited Internal Medicine Residency Program
- License/ Certification Requirements: Board Certification in Internal Medicine (or Board Eligible for new graduates)
