Responsibilities

• Deliver broad scope primary care
• Collaborate with an integrative team including APPs, behavioral health providers, clinical pharmacists, and case management
• Manage patient panels
• Provide inbasket management
• Coordinate patient referrals for ancillary services such as physical therapy and urgent care
• Provide high-quality outpatient care
• Support team-based practice
• Participate in care coordination
• Utilize available resources to enhance patient outcomes

Qualifications

Board-certified or eligible in Internal Medicine
