About the Role
Outpatient Primary Care for patients over age 65 with minimal call of 1:8.
Schedule: Monday–Friday, 8 AM–5 PM
Average patient load: 11–14 patients per day
Responsibilities
- Evaluate and treat center patients in accordance with standards of care.
- Coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.
- Act as an active participant and key source of medical expertise with the care team through daily huddles.
- Examine and analyze patient records, including allergies, medications, immunization status, and detailed medical histories (past medical, surgical, family, and social history).
- Conduct physical examinations, document findings, and diagnose medical conditions using patient history, examination results, and diagnostic tests.
- Administer, order, interpret, and act on diagnostic tests, such as blood work and imaging.
- Perform preventive and therapeutic procedures, including immunizations, injections, suturing, wound care, and infection management.
- Develop and implement individualized care plans, providing instruction and counseling to patients.
- Manage and monitor chronic diseases such as diabetes, hypertension, asthma, and obesity, ensuring long-term health outcomes.
- Meet with Lead Physician about quality of care, review of outcome data, and medical records issues.
- Prescribe medications within state guidelines and clinic regulations, and record patient progress.
- Review medication regimens for potential interactions, patient safety, and adherence to prescribed therapies.
- Provide general health education on proper diet, family planning, emotional well-being, and health maintenance.
- Offer in-house specialty consultations and services (e.g., psychiatric care) as appropriate to scope and training.
- Refer patients to specialists and collaborate on treatment plans as needed.
- Ensure timely and compliant completion of medical charts.
- Engage in community health initiatives such as health fairs or patient education workshops to identify trends and implement targeted interventions for high-risk populations.
Requirements
- Active license to practice medicine in Maryland.
- Current DEA certificate and up-to-date TB screening/vaccination.
- Board Certified or Board Eligible in Internal Medicine or Geriatrics.
- Fellowship training preferred.
- Experience in outpatient/ambulatory healthcare settings, including residency training.
- Knowledge of HEDIS, STARS, CAHPS, and/or HOS preferred.
- Medical Doctor with or without U.S. residency, with current U.S. work visa.

Compensation: Market salary plus bonus (~25% of salary); total Year 1 compensation reaches $315,000 or more. Partner track with zero buy-in.

About the Clinic
This suburban community health center provides easy access to care for senior patients, with a partnership to Medicare Advantage plans that fully cover primary care. Physicians maintain a smaller patient panel size while collaborating with outcomes-oriented specialists and hospitals. Each Primary Care Physician (PCP) leads a multidisciplinary team, including APNs, pharmacists, care associates, MAs, behavioral health support, quality-based coders, and referral coordinators. This model delivers a concierge-like experience with coordinated care, supported by analytics and tools, allowing physicians to spend more time with patients.
