Position Summary
This full-time position is part of an interdisciplinary team. Responsibilities include assessment, brief intervention and consultations with primary care, patient-centered health action plans, care coordination and care transitions with adults, youth, and families. You will be part of a team that will work toward better health outcomes by applying evidence-based practices. This is a dynamic program oriented toward healthcare transformation.
Requirements
- Master’s degree in Human Services, Health Sciences, or related field
- Licensed as a mental health therapist preferred
- Bilingual (English/Spanish) preferred
