Integrated Decision Team Facilitator Baby Cat

Position Summary

The Integrated Decision Team Facilitator and Wraparound Certified Coach will lead discussions to identify family needs and resources, establish safety and treatment plans, and ensure shared risk decision-making at critical junctures. They will also assist in developing and monitoring staffing schedules, and provide case management services including assessment, service planning, and client advocacy.

Qualifications

• Bachelor's Degree in Social Work or a related field from an accredited university
• Two years of experience working with the homeless population and/or persons struggling with severe and persistent mental illness and/or substance misuse disorders
• Certified as a Wraparound Facilitator in high fidelity Wraparound within 3 months of employment
• Must be willing and able to provide essential services during emergencies, pandemics, or disasters
