About the Role

This is a professional position providing facilitation of the Integrated Decision Team (IDT) for the Family Works (FW) program under the Housing First Division. The FW program offers an intensive wraparound approach to working with children from identified targeted schools in Pinellas County and their families to decrease homelessness and achieve stable and permanent housing potential.

FW includes assessing, linking, coordinating, and monitoring services from mental health, social, and educational aspects. The FW team collaborates and develops service plans in partnership with the family and works closely with them to identify services needed in the home to maintain a stable environment. Services are provided in-home or in the community, connecting families with resources and offering educational, financial, or other assistance as needed.

The IDT Facilitator is responsible for team coordination and coaching, focusing on clinical interventions, natural supports, community supports, and input from other community partners or professional agencies. The role ensures the youth and family have an individualized, strength-based treatment plan built collaboratively with each member of the IDT during sessions. The treatment plan, risk level, and clinical interventions are developed by the Integrated Decision Team, which includes the family, natural supports, and formal supports.

The IDT Facilitator ensures the youth and family are integral parts of the team and have ownership of the plan. No planning sessions occur without their presence, and youth are engaged as equal partners in creating systems for behavior change. The IDT Facilitator ensures services are culturally and linguistically competent and tailored to the unique culture of the child and family. IDT sessions are held in the community at a location convenient and comfortable for the family.

This is a field position. The IDT Facilitator must meet an annual client-facing care requirement of 1,410 hours, with achieving the minimum required weekly, monthly, and annual hours being an essential function of the role.

Directions for Living is an essential services provider. All team members must be willing and able to provide vital support and essential services during emergencies, pandemics, or other disaster situations.

Requirements

• Minimum of a Bachelor’s Degree in Psychology, Social Work, or a related field from an accredited university (Master’s degree preferred)
• At least two years of experience working with the homeless population and/or vulnerable children and families
• Must become certified as a Wraparound Facilitator in high-fidelity Wraparound within 3 months of employment
• Must become certified as a Wraparound Facilitator Supervisor/Coach within 3 months of employment
• Preference given to individuals currently certified as a Wraparound Facilitator Supervisor/Coach in high-fidelity Wraparound
