About the role

The Integrated Care Extender supports Integrated Care Coordinators in delivering care coordination and management services through performing various activities. This role involves general outreach, engagement, and follow-up with members, coordinating services and appointments, engaging in health promotion activities, and sharing information with the care manager and other members of the care team.

Responsibilities

  • Performing general outreach, engagement, and follow-up with members
  • Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation)
  • Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing
  • Sharing information with the care manager and other members of the care team on the member’s circumstances
  • Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services
  • Participating in case conferences
  • Supporting the care manager in assessing and addressing unmet health-related resource needs
  • Documenting all efforts within the care manager platform (ONEcare) timely
  • Participating in virtual team meetings and being required to attend in-person training/meetings as determined by leadership

Requirements

  • TN: Bachelor’s Degree in any discipline required. Bachelor’s Degree in human services related discipline preferred.
  • NC: At least 18 years of age, High School Diploma or equivalent, meets one of the below requirements, and has two years of paid experience performing the types of functions described in the “Extender Functions”, with at least one year of paid experience working directly with the Tailored Care Management eligible population.

Qualifications

  • Experience in relevant fields such as human services, psychology, or healthcare administration.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of Medicaid delivery systems and behavioral health conditions.

Pay

Compensation is commensurate with experience.
