Integrated Behavioral Health Clinician - Primary Care

Responsibilities

• Deliver evidence-based interventions to improve overall health for patients with a variety of emotional and behavioral problems.
• Meet with patients individually for assessments and provide recommendations to the primary care team to enhance their care plans.
• Provide on-going skills training and coaching to patients to improve health and functioning.
• Aid in the transition of patients requiring services not provided in the LMG clinic to care with community partners.

Qualifications

• Education: Master of Social Work, Master in Professional Counseling, Master of Family Therapy degree from a nationally accredited institution, fulfilling the requirements for licensure in the state of Oregon and/or Washington.
• Experience: Minimum of two years of clinical experience or healthcare related field.
• Licensure: Must hold a current valid Oregon and/or Washington License as LCSW, LICSW, LPC, LMHC or LMFT.
