Job Summary and Qualifications
As an Intake Social Worker III at Eastern Idaho Regional Medical Center, you will lead a team dedicated to creating positive patient interactions and caring for the multi-faceted wellness of those served. Key responsibilities include assessing and evaluating patients in crisis, screening patients at the Emergency Room, acting as a liaison to community mental health agencies, conducting assessments and evaluations, providing crisis intervention, and coordinating patient discharge and follow-up.
- Assess and evaluate patients in crisis to determine the best course of treatment.
- Screens patients at the Emergency Room and assesses and evaluates patients referred from other hospital units.
- Acts as liaison to the community mental health or other appropriate agencies to triage and quickly facilitate appropriate placement.
- Conducts assessments and evaluations in person and on the telephone.
- Streamlines and facilitates patient placement into the Behavioral Health department if warranted or refers to an appropriate community agency.
- Provides patients with adequate crisis intervention and safety planning if the patient is not being admitted.
Qualifications
- Basic Cardiac Life Support must be obtained within 7 days of employment start date.
- Nonviolent Crisis Intervention must be obtained within 60 days of employment start date.
- Masters Degree (LMSW/RCSW-I), Lic Master Social Worker/Reg Clin SW-Intern, or (LPC/LMHC) Licensed Professional/Mental Health Counselor, or (LCSW) Clinical Social Worker.
