Intake Patient Liaison (Home Health)

About the role

This a full time, in-person position based out of our Brooklyn, NY office.

Key Responsibilities

  • Review and respond to incoming patient referrals, ensuring timely processing and appropriate prioritization.
  • Verify patient eligibility, including insurance benefits and authorization requirements.
  • Coordinate and hand off qualified cases to the Intake team for onboarding and scheduling.
  • Maintain ongoing communication with LHCSAs and referral sources, providing timely status updates on patient cases.
  • Track referral progress to ensure cases are opened efficiently and transitioned to Clinical Operations without delays.
  • Collaborate closely with Intake and Clinical Operations teams to ensure continuity of care and accurate information flow.
  • Identify and escalate issues or delays in the intake process to appropriate stakeholders.
  • Maintain accurate and up-to-date documentation in internal systems.

Qualifications

  • 2+ years of experience in home care, healthcare intake, care coordination, or a related field.
  • Strong understanding of insurance verification, eligibility, and authorization processes preferred.
  • Experience working with LHCSAs or similar referral networks is a plus.
  • Excellent organizational and multitasking skills with strong attention to detail.
  • Effective communication skills with the ability to collaborate across departments.
  • Skilled in interpersonal relationships.
  • Verbal and written communication skills.
  • Proficiency in EMR systems and Microsoft Office or Google Workspace.
  • Demonstrates professional behavior, attitude and flexibility.
  • Preferred Skills: Experience in home care, Ability to manage high referral volume in a fast-paced environment, Strong problem-solving skills and ability to navigate complex case scenarios, Customer-service mindset with a focus on patient and partner experience.

Working Conditions

  • Able to read 12 point or larger type.
  • Must be able to hear adequately with no more than an amplifier on the phone and to speak in a manner understood by most persons.
  • Able to lift, bend or stoop in order to carry out above duties.

Pay

$23.00-$25.00/hrly
