About the role
We are seeking a licensed, experienced social worker to lead our Intake Department as a Dialysis Intake Manager. This role is ideal for a professional who combines strong clinical assessment skills with leadership experience and a deep understanding of care coordination.
Responsibilities
- Lead and manage daily intake operations across clinics, ensuring timely and efficient patient admissions
- Conduct or oversee psychosocial and care needs assessments for incoming patients
- Guide patients and families through the intake process, addressing barriers such as insurance, transportation, and social support
- Collaborate closely with physicians, nurses, social workers, and administrative teams to coordinate care plans
- Ensure compliance with CMS, state regulations, and organizational standards
- Supervise, coach, and develop intake team members, fostering a compassionate and accountable culture
- Track intake metrics, referral trends, and conversion rates to improve processes and patient access
- Partner with hospital discharge planners and community providers to strengthen referral relationships
Requirements
- Licensed Social Worker (LCSW or LMSW preferred)
- Bachelor's degree required, Master of Social Work (MSW) strongly preferred
- 5+ years of healthcare experience, with emphasis in social work, case management, or care coordination
- Leadership or supervisory experience required
- Dialysis or chronic disease management experience highly preferred
- Strong knowledge of insurance, discharge planning, and patient resource coordination
- Excellent interpersonal skills with the ability to support patients and families
- Experience working with diverse populations and complex social needs
- Proficiency in EHR systems and healthcare operations
