About the Role

Under the supervision of the Manager of Patient Assessment and Intake, the Intake Coordinator facilitates and coordinates the referral process to ensure timely and appropriate review and dissemination of information.

Responsibilities

  • Patient Population
    • Engages in population-appropriate communication, demonstrating knowledge of growth and development milestones and tasks.
    • Provides clear instructions to patients and families regarding treatment, involving them in assessment, initial treatment, and continuing care.
    • Identifies physical limitations of patients and deploys necessary interventions.
    • Recognizes and responds appropriately to patients and families with behavioral health problems.
    • Interprets population-related data and plans care accordingly.
    • Identifies and responds to unique psychological needs or those associated with religious or cultural norms.
    • Performs treatments, administers medication, or operates equipment safely.
    • Recognizes and responds to signs or symptoms of abuse or neglect.
  • Intake Process
    • Accepts demographics, clinical, and payer information on patients referred via ECIN, fax, or telephone.
    • Develops and maintains an effective computerized system for tracking all referrals, admissions, and denials.
    • Maintains the Daily Patient Referral Board in real-time, tracking each referral from receipt to final disposition.
  • Documentation
    • Collects, analyzes, and reports data related to referral intake information.
    • Documents all referrals and associated decisions regarding admissions and denials.
    • Completes all necessary facility documentation to ensure efficient exchange of information from referring providers.
    • Documents in MIDAS and Excel spreadsheets.
  • Communication
    • Serves as the central information resource for all patient referral and assessment activity.
    • Facilitates communication between the Assessment Liaison Team and Admitting to assist with bed management for incoming patients.
    • Answers calls and inquiries for UC Medical Center.
  • Financial Duties
    • Ensures referrals are received in Admitting along with the Utilization Nurse for timely insurance verification.
    • Seeks and maintains current information related to third-party payers.
    • Refers self-pay patients to appropriate finance personnel for payer source and financial aid assessments.
  • Performs other duties as assigned.

Requirements

  • High School Diploma or GED required; Associate's Degree preferred.
  • Notary certification preferred.
  • Minimum of 2 years of equivalent experience required.
