Job Qualifications

  • Education: High School Diploma or GED
  • Experience: One year of data entry experience. One or more years of intake experience desired.
  • Specialized Knowledge and Skills: Basic medical terminology, Computer Skills including Microsoft Office, MS Excel, MS Word
  • Personal Traits, Qualities and Aptitudes: Knowledge of physician, hospital, skilled nursing facility, case management and discharge planning service needs, Ability to effectively communicate with multiple departments in a timely manner, Ability to multi-task and adjust to a fast-paced work environment with high demands, Ability to handle stressful situations and deadlines, Ability to demonstrate strong communication skills, Promotes team efforts and works well with others, Recognizes the rights and responsibilities of the company and patient confidentiality, Ability to redirect priorities on short notice.

Primary Job Functions

  • Input Patient Referrals: Enter patient referral documentation and information for all Home Health and Hospice Agencies within a timely manner.
  • Review Clinical Information: Notify appropriate Care Transition Coordinator of any additional information needed to complete the patient’s chart.
  • Notify Appropriate Staff: Notify the appropriate Care Transition Coordinator, Clinical Intake Coordinator, Director of Business Development, Branch Administrator, and Clinical Manager of receipt of referral.
  • Manage Electronic Referral Systems: Accept or decline referrals timely within the systems, notify CTC’s of referral status, and provide professional and helpful communication.
  • Completing Daily Intake Report: Add all information needed to the daily intake report and send to the appropriate staff.
  • Accepting and Declining Referrals: Allow acceptance or decline based on insurance and service area, move self-pay requests to review status.
  • Customer Service Skills: Provide accurate, valid, and complete information, advise on company information and services, and field customer questions to the appropriate department.
  • Administrative Duties: Promptly attend meetings, complete assignments, send daily intake report, answer emails, follow up with CTC’s, submit timesheets, and perform other duties as assigned.
