Intake Coordinator-Home Health

Job Summary

Responsibilities

• Cook up intake of home health referrals and make sure they get processed promptly.
• Prioritize referrals, manage pipelines, and predict how many people might need help.
• Make sure everything follows rules about privacy and safety.
• Check if patients are eligible, have good insurance, and meet the rules for getting help.
• Keep track of patient information in special computer programs and make sure it's all correct.
• Talk to families, doctors, and money people about what's happening with the patients.
• Show how many patients are coming in and how long it takes.
• If there are tricky situations, tell the right people about them.
• Help organize the work for the team and make sure everyone knows what to do.
• Find ways to make the work go faster and better.
• Teach new people how to do the job.

Qualifications

• Know a lot about keeping patient information safe and following rules.
• Have experience doing intake for home health or similar jobs.
• Be good at using special computer programs for patient information and making sure it's all correct.
• Be able to guess how many people might need help and keep track of it.
• Be good at checking if patients are eligible and have good insurance.
• Be good at talking to families, doctors, and money people.
• Be organized and good at figuring out priorities and schedules.
• Be able to look at how well the job is going and find ways to make it better.
• Be able to teach new people how to do the job.
