Intake Coordinator
Position Title to be filled as soon as possible
Required Minimum Qualifications
At least 2 years of experience in an administrative support capacity, 1 year of experience in a healthcare or social services setting.

Responsibilities and Initiatives
- Serve as point of contact for social workers on referrals, providing internal and external updates on referral status.
- Create and initiate client file, ensure referrals are documented correctly and in a timely manner into a database, confirming that our Guest Services Associates, Nursing and Social Services team have been updated.
- Communicate effectively with team about referrals and admissions.
- Build trusting relationships with hospital partners and community partners.
- Deliver excellent customer service and exercise good judgment with a compassionate, positive attitude while managing a population of clients who may be experiencing difficult life events, such as homelessness, unemployment, addiction recovery, health issues and/or poverty.
- Support marketing efforts that help promote services, educate stakeholders, and identify new opportunities (i.e., conducting in-service trainings and site visits, following up on marketing leads, preparing marketing materials and presentations, retrieving success stories, coordinating webinars, updating databases and spreadsheets, and creating reports and dashboards).
- Maintain guest census roster and ensure intake/discharge.
- Handle insurance care coordination for our guests in conjunction with Social Services department.
- Provide input and evaluation of programming for quality improvement purposes.
- Maintain confidentiality of patient information.
- Work collaboratively with other members of the team, including active participation in department and agency staff meetings and trainings.
- Attend and provide information on guest referral, intake, and discharge status during board review meetings.
- Project a positive professional image and adhere to organization dress code.
- Flexibility to adapt to schedule changes to meet the needs of the facility.
- Aid other NHF facilities as assigned by supervisor when the need arises.
- Complete additional duties and assignments as required to meet the needs of the facility, notwithstanding primary roles.
- Always follow NHF policies and procedures.
Qualifications
- Strong proficiency with Microsoft Suite which includes Word, Excel, and Outlook.
- Excellent in multi-tasking; have strong organizational and time management skills.
- Experience in medical office/recuperative care setting (preferred).
- Familiarity with HIPAA (preferred).
- Have friendly and service-oriented mindset. Be people first and be driven by empathy.
- Bilingual in a language that is commonly spoken in Los Angeles, other than English. (Preferred, not required).
