About the role
The Home Health & Hospice Intake Coordinator is a critical operational role responsible for managing the full intake process for both home health and hospice services. This position ensures referrals are processed accurately, benefit verification is completed efficiently, and documentation meets regulatory and payer requirements to support compliant admissions. The coordinator’s work directly influences start-of-care timeliness, revenue capture, patient experience, and referral partner satisfaction. The role requires strong judgment, the ability to navigate complex payer rules and clinical documentation, and coordination across multiple departments. By supporting two highly regulated service lines, the Intake Coordinator contributes significantly to organizational performance, operational throughput, and overall quality of care transitions.
Requirements
- High School Diploma (required)
- Bachelor’s Degree (preferred)
- 2-4 years of experience in home health, hospice, utilization management, or post-acute intake (required)
- Familiarity with Medicare / Medicaid regulations in post-acute care (preferred)
