About the role
This is a full-time, hourly position reporting to the PAS Program Director. The Intake and Consumer Services Coordinator is responsible for Personal Assistant Services (PAS) consumer intakes and referrals, and assists with the coordination of consumer services.
Schedule
Days and hours of work are Monday through Friday, 8:00 AM to 4:30 PM. Occasional evenings and weekends may be required.
Responsibilities
- Conduct consumer intakes and referrals, including verification of eligibility, both phone and in person, while maintaining confidentiality and respect to consumer needs
- Triage for appropriateness for PAS services
- Provide education to consumers and caregivers on eligibility for services and systems navigation
- Complete required documentation in PAS databases; generate reports on related activities and outcomes as designated
- Actively participate in the PAS Coordination team meetings and other meetings as requested
- Communicate effectively with internal teams and external partners
- Attend community events to support recruitment and engagement efforts, acting as an ambassador for Independence First
- Advocate for consumer needs with outside partners where appropriate
Work Environment
The work environment is generally performed in an office setting with minimal to moderate noise level.
Preference given to candidates who are bilingual in English, Spanish, Hmong, or Arabic (not required).
