Responsibilities
- Upon consultation, conducts a comprehensive assessment of the patient's behavioral, psychological and socio-economic needs.
- Attends Care Planning or Discharge Planning rounds to facilitate communication and the importance of the patient and family's goals.
- Participates in end of life discussions and advance care planning.
- Bases social work actions by code of ethics according to National Association of Social Workers (NASW).
- Documents measurement and evaluation of social service consultation and satisfaction outcomes as patient progresses towards desired goals.
- Provides leadership to departmental staff.
- Performs other duties as assigned.
Requirements
- Minimum of three years clinical Social Work in a hospital, Skilled Nursing facility, Home Health or Hospice setting.
- Demonstrates effective interpersonal, organizational and motivational skills.
- Demonstrates ability to function as a team player.
- Proficient in written and verbal communication.
Qualifications
- Masters Degree in Social Work or related field required.
