Inpatient RN Registered Nurse Care Coordinator I - Care Transitions

About the role

Functions as part of an interdisciplinary team to maintain continuity of patient care for identified patient populations from admission to discharge, utilizes appropriate criteria for inpatient care and plans timely discharges. Responsible for coordinating patient's clinical plan of care guided by Patients/Families, Physician Providers, and other members of the inpatient team while adhering to guidelines and contracts from regulatory bodies and insurers.

Responsibilities

• Maintain continuity of patient care for identified patient populations from admission to discharge.
• Utilize appropriate criteria for inpatient care and plan timely discharges.
• Coordinate patient's clinical plan of care guided by patients/families, physician providers, and other inpatient team members while adhering to guidelines and contracts from regulatory bodies and insurers.

Requirements

• Two (2) years of nursing experience.
• Currently licensed to practice as a Registered Nurse by the state of employment or hold a multistate RN license through the Interstate Nurse Licensure Compact.
• AHA BLS certification prior to the end of the orientation period.

Qualifications

• BSN preferred.
• Previous experience in case management, care coordination, outcomes management, or discharge planning preferred.
• Certification in case management from a nationally recognized organization preferred.
