Transitions of Care Nurse

Role: RN Medical Transition Management Responsibilities Coordinate discharge planning, medication reconciliation, and follow-up care to ensure safe patient transitions across care settings.Educate patients and families to improve understanding and reduce readmissions.Collaborate with physicians, case managers, and support staff to develop personalized care plans.Address barriers to access and support seamless continuity of care. Qualifications & Skills: Registered Nurse (RN) licensure.Strong communication and patient education skills.Ability to work effectively within a team-based, compassionate environment.Knowledge of discharge procedures, medication management, and care coordination.
