About the role
The Rehab Liaison is responsible for developing and maintaining referral relationships, facilitating inpatient rehabilitation admissions, and completing clinical pre-admission assessments to ensure appropriate patient placement. This role serves as a liaison between the rehabilitation unit and referral sources while supporting growth, compliance, and coordination of care.

- Facilitate inpatient rehabilitation admissions
- Complete clinical pre-admission assessments
- Serve as a liaison between the rehabilitation unit and referral sources
- Support growth, compliance, and coordination of care
Requirements
- Bachelor's degree or equivalent clinical experience
- Clinical licensure (RN, LPN, PT, PTA, OT, COTA, SLP)
- 2–3 years healthcare experience
- PREFERRED: Rehab admissions or business development experience
