Inpatient Pharmacist

About the role

Oversee and ensures a safe and efficient drug distribution system for patients, including clinical functions. Supervises the day-to-day operations of the pharmacy. Supervises the work of pharmacy supportive staff. Monitors the day-to-day management and acquisition of medications and supplies. Facilitates problem solving of pharmacy issues. Assists with department/division quality improvement activities. Provides feedback on performance for all pharmacy personnel.

Responsibilities

• Ensure daily and weekly computer maintenance functions performed (input as assigned).
• Offer suggestions to improve services and viable options to resolve problems.
• Monitor, review, interpret and dispense medication orders in accordance with professional and legal standards and practices.
• Interpret medication and treatment orders in relation to patient's health status.
• Maintain a computerized patient profile system in order to monitor patients' medication therapy for safety and effectiveness.
• Schedule the administration of medications to provide optimal effect.
• Evaluate prospectively patient medication records for DUR and DUE.
• Compound/supervise the compounding of prescriptions and/or intravenous admixtures.
• Take appropriate action in response to computer-identified interactions.
• Demonstrate an understanding of reimbursement guidelines and issues and incorporate them appropriately into medication dispensing activities.
• Ensure current billing of patient medications.
• Check all medication selections prior to dispensing to patient.
• Develop and implement clinical pharmacy activities using outcome based guidelines and MercyOne policy.
• Monitor patient response to medications and effectiveness of therapy.
• Evaluate patient response to medications and effectiveness of therapy to include diagnostic/lab results.
• Communicate with and make recommendations to physicians and other health team members regarding patient response, effectiveness of therapy and/or dosing levels and monitor as appropriate.
• Monitor and/or dose medications and participate in patient rounds as requested by physicians.
• Operate the clinical pharmacy programs within the protocols and guidelines.
• Establish and maintain a professional relationship with the patient.
• Intervene on behalf of the patient as appropriate to access appropriate care within the health system.
• Ensure the patient has all the supplies, information, knowledge and skills to carry out the drug therapy plan.
• Demonstrate competency in performing required lab and diagnostic testing as outlined by clinic protocols.
• Complete documentation. Document patient assessment, plan of care, interventions and evaluation of plan accurately and legibly.
• Ensure completion of all documentation required by regulatory agencies and corporate policy.
• Provide clinical and drug information support to healthcare professionals and patients.
• Provide clinical medication information to medical and nursing staff.
• Provide a comfortable and professional atmosphere for patient.
• Counsel patients and/or designated individuals on appropriate use and safety of prescribed medications.
• Respond to drug information requests and provide accurate drug information in the course of daily practice.
• Provide effective patient education in multiple ways - verbal, written, pictorial, demonstration - based on the needs of the patient and/or caregiver to ensure proper use of medications.
• Prepare and present drug therapy-related educational programs/in-services to patients and other health care providers as needed.
• Coordinate and manage student educational experiences.
• Maintain medication and supplies inventory (retail only). Assess medication and supply inventory levels daily. Consider contractual agreements prior to ordering. Obtain appropriate order authorization, i.e., P.O. number. Order medications and supplies from appropriate source (wholesaler vs. direct). Receive and check in medications and supplies. Complete documentation of inventory received. Research discrepancies and process returns and credits. Code and bill invoices to appropriate departments. Maintain necessary purchasing documentation for business and regulatory needs.
• Initiate and complete sales transactions (retail only). Identify merchandise type and choose correct cash register key. Operate cash register and MasterCard/Visa machine. Receive payment and return correct change to customer. Assure cash register has adequate amount of change. Print cash register and credit card reports, reconcile each cash register daily and complete reconciliation form. Prepare deposit and deliver to designated point daily.
• Complete pharmacy department paperwork. File paperwork on dispensed prescriptions. Deliver/distribute pharmacy paperwork or mail prescriptions.
• Report medication errors to appropriate personnel.
• Accurately complete pharmacy data collection reports.
• Assist in maintaining an organized and clean pharmacy.
• Collect, analyze, interpret and apply age-specific data.
• Perform tasks/procedures, use equipment in an age appropriate manner.
• Communicate and work with others in an age appropriate manner.
• Adhere to all safety, infection control and colleague health policies and procedures.
• Attend and complete employer required safety programs.
• Appropriately and consistently use the personal protective equipment required of the job.
• Know and can demonstrate departmental emergency procedures for fire, disaster and other emergency situations.
• Report and remove unsafe equipment/furniture.
• Ask for assistance when needed to assure safety for all involved.
• Maintain a safe working environment and practice safe working habits.
• Maintain a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health's Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

Qualifications

• Bachelor's Degree: Graduate from accredited college of pharmacy. PharmD preferred.
• Current Iowa Pharmacist license.
• No prior experience required.
