Overview

Care Management staff assess patients throughout hospitalization to identify medical, psychosocial, functional, financial, and environmental factors that may affect treatment, recovery, or discharge. In collaboration with patients, families, providers, and community partners, the team develops individualized care transition plans that align with patient goals, treatment preferences, available support systems, and post-acute care needs.

Services include discharge planning, care coordination, psychosocial assessment, patient and family education, advocacy, resource referral, and facilitation of post-acute services such as home health, rehabilitation, skilled nursing, hospice, and community-based support. The department also assists with addressing social determinants of health, including financial, insurance, housing, and transportation needs. The goal of the Care Management Department is to promote quality outcomes, patient safety, effective resource utilization, and successful transitions of care while reducing barriers to discharge and preventable readmission.

Schedule

  • Full-Time: Monday - Friday (either 4/10-hour or 5/8-hour shifts)
  • PRN or Part-time available as needed
  • Days

Qualifications

  • Education: Bachelor's or Master's in Social Work
  • Licensure: Current license in Social Work (LMSW, LCSW), or LPN/RN in the State of Missouri (Preferred)
  • Experience: Any experience in acute care hospital setting (Preferred)

Skills & Knowledge

  • Strong verbal and written communication and facilitation skills
  • Strong assessment skills, time management skills, critical thinking skills
  • Advanced computer skills
  • The assessment, care and treatment provided by Social Worker II will be consistent with the specific age-related needs of the patient. Is competent to care for patient age groups including neonatal/infant, child, adolescent, adult and geriatric adult patients
  • Skills and knowledge of human behavior and clinical aspects of medical Social Work
  • Ability to interact effectively with a variety of people and situations at all levels of the organization
  • Valid Driver's license and current Motor Vehicle Registration
