About the role
The Department of Care Management utilizes the Team Leader model to enhance operational and clinical effectiveness and ensure seamless management of day-to-day activities for staff engaged in discharge planning, care coordination, social work, and capacity management.
Responsibilities
- Serves as clinical expert and first-level consultant for staff within a designated area to address patient care issues and discharge barriers, advising staff and including departmental leadership as necessary.
- Acts as point of contact and mentor for members of the designated team.
- Monitors daily census to ensure appropriate staffing and distribution within the designated team.
- Schedules and conducts weekly team meetings/huddles.
- Manages PTO and holiday staffing for the designated team.
- Improves and sustains patient flow, length of stay, and capacity management.
Requirements
- Master of Social Work degree from a CSWE-accredited school of social work with a minimum of two years of case management experience, or
- Bachelor of Science in Nursing from an accredited college or university with a minimum of two years of case management experience.
- If a Registered Nurse, current licensure as a Registered Nurse by the NC Board of Nursing is required.
- Certification in Case Management required within two years of hire.
- One to three years of experience in case management/discharge planning, preferably in acute care case management.
Pay
$72,259 – $105,328
Schedule
Monday–Friday, 8:00 am–4:30 pm; no weekends or holidays.
