About the role
Provides direct nursing services and case management for infusion therapy in patients' homes, clinics, or alternative sites in Oregon and southwest Washington. Collaborates with healthcare providers, patients, caregivers, physicians, and payors to ensure quality care. The role emphasizes supporting vulnerable populations and fostering professional growth, with a focus on inclusive, patient-focused culture.
Responsibilities
- Coordinating infusion treatments
- Delivering patient-centered care
- Ensuring compliance with clinical standards
- Maintaining communication across multidisciplinary teams to facilitate effective treatment plans
Qualifications
- Oregon RN license
- Valid licenses in care delivery states
- BLS certification within 30 days of hire
- Preferred: at least one year of experience in acute hospital, infusion, or home health care
