Infusion Patient Care Coordinator - Licensed Practical Nurse (LPN)
The Position
The Infusion Patient Care Coordinator is responsible for:
- Data entry of clinical information into clinical management system for billing
- Clinical scheduling and associated tasks for the nursing department
- Inter-agency care coordination
- Responding to all incoming calls
- Promoting agency programs
- Providing information and processing referrals
- Collaborating with prescribers to facilitate payor denial appeals
- Acting as a single point of contact for internal and external customers
- Serving as the main point of contact for the nursing department
- Coordinating and planning scheduling activities for the nursing department
- Demonstrating knowledge of the geographical area for efficient scheduling and coordination
- Clearly communicating patient needs for external agency support
- Collaborating with internal departments and external partners
- Serving as a key touchpoint for enhancing the in-clinic patient experience
- Maintaining accurate and efficient schedules
Schedule
Full time, 40 hours per week, from 8:30am - 5pm - in-office
Requirements
- High school diploma or equivalent required
- Licensed Practical Nurse/Licensed Vocational Nurse License preferred
- Prior healthcare scheduling experience required
- Home infusion experience required
- Ability to work on-site and interact directly with patients in a clinical setting
- Strong communication skills
- Basic computer skills including Microsoft Excel, Word, Outlook
- CPR Plus application experience preferred
Qualifications
- Knowledge of the geographical area for efficient scheduling and coordination
- Ability to prioritize and multitask
- Experience with EMR and EHR systems
- Experience with clinical data entry
- Experience with patient education and care coordination
